Dr. Ezzo is a dedicated professional committed to providing quality mental health and substance abuse services. It is his overall goal to enhance the quality of life for individuals and families. Dr. Ezzo belives that all people are valuable and unique and should be treated with dignity and respect. While recognizing the potential for change, an assessment of the client’s emotional, physical, spiritual, and life experience is provided in a caring environment. The growth of the individual is promoted through a course of treatment developed and executed in a timely and cost effective manner.

Ethical Stance

Dr.Ezzo is dedicated to serving the best interest of each client.
Dr.Ezzo will not discriminate between clients or professionals based on age, race, creed, disabilities, handicaps, preferences, or other personal concerns.
Dr.Ezzo maintains an objective and professional relationship with each client.
Dr.Ezzo respect the rights and views of other mental health professionals.
Dr.Ezzo will appropriately end services or refer clients to other programs when appropriate.
Dr.Ezzo will evaluate our personal limitations, strengths, biases, and effectiveness on an ongoing basis for the purpose of self-improvement. He will continually attain further education and training.
Dr. Ezzo holds respect for various institutional and managerial policies, but will help improve such policies if the best interest of the client is served.

Involving children in your treatment

It may be a good idea for your children to participate in some stage of your therapy. This form must be read, understood, and signed before that should happen.

Cancellation Policy

We set aside a total of one hour for your appointment. There are many reasons for you to recognize this hour as an important priority. I plans this hour for you. How important is it to you to reach your goals? If you are inconsistent or attend sessions sporadically, this breaks the pattern or momentum of therapy and your progress.

You must agree to the No Show and Cancellation policy before therapy session can begin. With few exceptions a $35.00 cancellation will apply.
